Last updated: July 1, 2025. These Terms of Service govern your access to and use of Sculpt’s telehealth platform and related services. By using Sculpt, you agree to these terms. If you do not agree, do not use our services.
1. Medical Disclaimer
Sculpt provides a telehealth platform that connects patients with licensed healthcare providers. Nothing on this website constitutes medical advice. A licensed provider must evaluate your health history before any prescription is issued. Compounded medications dispensed through Sculpt are not FDA-approved and are not generic or FDA-approved equivalents of Ozempic®, Wegovy®, Mounjaro®, or Zepbound®.
2. Prescription Requirement
All medications require a valid prescription from a licensed U.S. healthcare provider. Completing our intake questionnaire does not guarantee a prescription. Providers in our network retain full clinical discretion and may decline to prescribe based on your health history or other clinical factors.
3. Refund & Cancellation Policy
You may cancel your subscription at any time. If a provider determines treatment is not appropriate after you have completed payment, you will receive a full refund. Refunds are not available once medication has been shipped. For refund questions, contact support@joinsculpt.com.
4. Use of Platform
You agree to use Sculpt only for lawful purposes. You must be at least 18 years of age and located in a state where Sculpt offers services. You are responsible for providing accurate health information. Misrepresentation of your health history may result in termination of your account.
5. Intellectual Property
All content on this website, including text, graphics, logos, and software, is the property of Sculpt Health, Inc. and is protected by applicable copyright and trademark law. You may not reproduce or distribute any content without our prior written consent.
6. Limitation of Liability
To the maximum extent permitted by law, Sculpt shall not be liable for any indirect, incidental, or consequential damages arising from your use of our services. Our total liability to you shall not exceed the amount you paid for services in the prior 30 days.
